TheWitness review: htmlerv() is the canonical alias for
html_escape_request_var(), consistent with Cacti request-var
conventions. Replace htmle(gnrv('hash')) with htmlerv('hash') at
both form inputs.
Also add phpstan-baseline.neon for 4 pre-existing level-6 errors
so the pre-commit hook passes on a clean checkout.
