* Issue #1723 - die() messages should include \n for better logging

This corrects issue #1723 where die() messages appear in log files without a new line character causing logs to appear corrupted.

* Replace \n with PHP_EOF to be platform independant.

If you are running in development, you have to issue these two commands
rename table settings_graphs to settings_user;

cigamit 0ed731cd8a Improve debug_backtrack() logic and consolidate use 
Remove debug_backtrace() from a few areas and consolidate into a single
call.  Improve backtrace logging to include HTML output as an option.
