Timur Tukaev 6f756fa584 docs(community): add a Role nomination issue template

Both Hidora maintainers were promoted through a well-run public vote in #2343
and #2344: a written rationale, representative contributions, a 5-day window,
+1/0/-1 from the maintainer roster, the nominee confirming acceptance, and
explicit next steps. None of that shape was written down anywhere afterwards, so
the next nomination has to reinvent it, and the Reviewer rung added in July has
no filed nomination at all.

This template is that issue turned into a form, covering both roles. It states
what the ladder requires, asks for an honest note when a requirement is not met
rather than a quietly unticked box, and records that the promotion itself lands
through a pull request against CODEOWNERS or MAINTAINERS.md — the three current
entries were added by direct commit, which the ladder does not describe.

The config.yml routing sends governance to the community repo; nominations are
the exception, because MAINTAINERS.md, CODEOWNERS and CONTRIBUTOR_LADDER.md all
live here and this is where the two prior votes happened.
