Move the external agentic-qe-fleet entry from v3.15.10 (e5814a2f) to
v3.15.11 (221d6de15c73bb385961e4e649d8fcc1f5dc2dea). The plugin folder
changed only its version and its MCP pin (agentic-qe@3.15.11). Updates
the README links, the external-plugins test fixture and ADR-484.
