reasoningbank-adapter: generatePatternId() appended Date.now(), so a repeated successful trajectory merged into its existing pattern only when both calls fell in the same millisecond. Otherwise it created a duplicate pattern with usageCount 1 (the adapter never learned from repeats), and the "update existing pattern" test failed intermittently in CI. The id is now a pure function of the goal (slug + FNV-1a of the full normalized goal). A new test pins Date.now to two different values; it fails on the old code. v3-ci Build V3 (macos-latest): when the better-sqlite3/hnswlib-node prebuild download times out, node-gyp compiles from source and macos-latest's Python 3.12 has no distutils.
