This change loads the Monitor CSS in the header instead inline in the
HTML code. This will eliminate the annoying flash you get with the
screen every time you refresh the page.
This is a fix for issue #61 where monitor_list was not being used to notify of reboots. It also includes a feature request to override the threshold alert list inclusion so only monitor_list is used.
This fixes the mute problem described in issue #57 where the audio would constantly be playing and unmutable.
It also addresses other issues with insert audio links to files that do not exist, plus removes some duplicated code.
