Minor update to monitor to properly support Cacti 1.2

This change loads the Monitor CSS in the header instead inline in the
HTML code. This will eliminate the annoying flash you get with the
screen every time you refresh the page.

Fix for Issue #28
...
issue#28 - Added ability to add a default for the monitoring option so that when new devices are added, they get the default value.
One slight hack was to try and get the 'id' request variable, as it isn't passed when calling the configuration field setup meaning you can't tell if you are editing a new or existing item.

Modified the monitor_scan_dir() function so the file monitor sounds files are sorted in the list, but maintained the 'None' option at the top.
