docs(plans): fix two pre-rebase hashes and the drifted Server query-site count

The plan-76 completion record cited a75f435f (clipboard) and 3ac566cb (first
poller wave) — pre-rebase hashes unreachable from HEAD; the landed commits
are e1a5a5e1 and 1503c493. The plan-77 record's '98 query sites' for the
Server SoftDelete tail measures 99 today, so it is recorded as approximate.
