Juan DenisandClaude Fable 5 5448694c8d refactor(app): SERVERKIT_SKIP_BACKGROUND reads through the env door ...

The last hand-rolled truthiness read in the app factory: it accepted only
the literal '1', invisible to the env-door ratchet's pattern. env_bool()
keeps '1' working (the updater's contract) and adds the door's documented
true/yes/on spellings.
